Comprehensive Discharge Planning Program
- Multi-disciplinary discharge planning process coordinates discharge and after-hospital home and community-based services.
- Parent education including infant CPR training.
- Rooming-in available for parents to gain confidence in their skills to care for and give any needed special treatments to the infant in preparation for the discharge home.
- Referrals to community agencies and follow up services, including to children's rehabilitation programs at Blythedale Children's Hospital and St. Mary's Hospital.
- Referrals to primary care pediatricians and pediatric specialists as needed.
- Home monitoring and specialized equipment if needed.
- Home health nurse visitation (Montefiore Home Health Care, Visiting Nurse Service) and extended home nursing.
- SYNAGIS shots for very premature babies at risk for RSV infection.
- Breastfeeding and lactation support.
- Referral for early intervention services for infants demonstrating a developmental delay.
