Comprehensive Discharge Planning Program

  • Multi-disciplinary discharge planning process coordinates discharge and after-hospital home and community-based services.
  • Parent education including infant CPR training.
  • Rooming-in available for parents to gain confidence in their skills to care for and give any needed special treatments to the infant in preparation for the discharge home.
  • Referrals to community agencies and follow up services, including to children's rehabilitation programs at Blythedale Children's Hospital and St. Mary's Hospital.
  • Referrals to primary care pediatricians and pediatric specialists as needed.
  • Home monitoring and specialized equipment if needed.
  • Home health nurse visitation (Montefiore Home Health Care, Visiting Nurse Service) and extended home nursing.
  • SYNAGIS shots for very premature babies at risk for RSV infection.
  • Breastfeeding and lactation support.
  • Referral for early intervention services for infants demonstrating a developmental delay.
